HOW TO CHANGE YOUR LEGAL NAME ON YOUR ARIZONA CCW PERMIT
If your legal name has changed due to marriage, divorce, or a court-ordered name change, you can request an updated Arizona CCW permit card through the Arizona Department of Public Safety (AZDPS). Your permit number and expiration date will remain the same. AZDPS will issue a replacement permit card showing your updated legal name.

Four Ways To Request A Name Change
There are four ways to submit a legal name change request for your Arizona CCW permit: online, through our Concierge Replacement Service, by mail, or by dropping off your application at AZDPS in Phoenix.
If you do not know your Arizona CCW permit number, you may contact the AZDPS Concealed Weapons Permit Unit at 602-256-6280. An agent can locate your permit information after verifying your identity.
When completing your request, enter your current information, including any changes to your address, telephone number, or other personal information since your permit was issued.
For a legal name change, select “Legal Name Change” as the application type. The AZDPS replacement permit fee is $10. If you submit your request online, additional credit card processing and service fees apply. The current online total is $15.80.
Once AZDPS approves your request, your replacement permit card will be mailed to you. Your permit number and expiration date will remain the same.

OPTION 1: To submit a legal name change request online, click the button below and complete the AZDPS online application with your current personal information.
Be prepared to upload the required documentation. Each individual file may be no larger than 5 MB.
- Proof of identity — Government-issued photo identification, such as a driver’s license, state-issued identification card, or passport.
- Proof of legal name change — Certified court documents, marriage license, or divorce decree.
Select “Legal Name Change” as your application type. The AZDPS fee is $10, plus online credit card processing and service fees, for a current total of $15.80.
After completing payment, download and save your confirmation and paid receipt for your records. Please allow AZDPS up to 45 days to process your name change application. You will receive an approval confirmation by email once your request has been processed.

SUBMIT LEGAL NAME CHANGE APPLICATION BY MAIL
OPTION 3: If you prefer to mail your legal name change request, download and complete the CCW Permit Replacement Request form below. Select “Legal name change” as the reason for your request and complete all applicable fields.
Be sure to include:
- A copy of your government-issued photo ID
- A copy of your court document, marriage certificate, or other accepted documentation showing your legal name change
- A $10 cashier’s check, money order, or certified check payable to AZ DPS
Mail your completed application, supporting documents, and payment directly to AZDPS at the address below. Be sure to include sufficient postage. Because the application is mailed to a P.O. Box, there is generally no need to pay for overnight shipping. You may choose a mailing option that provides tracking so you can confirm delivery.
Please allow AZDPS up to 45 days to process your request. Incomplete information or missing documentation may delay processing.

Important: Once you receive your replacement permit card showing your new legal name, you are responsible for returning your previous permit card to AZDPS. Your permit number and expiration date will remain the same.

OPTION 4: You may visit the Arizona Department of Public Safety (AZDPS) Public Services Center in Phoenix to submit your legal name change request in person. Before visiting, download and complete the CCW Permit Replacement Request form and select “Legal name change” as the reason for your request.
Be sure to bring:
- Your completed CCW Permit Replacement Request form
- A copy of your government-issued photo ID
- A copy of your court document, marriage certificate, or other accepted documentation showing your legal name change
The office is open Monday through Friday from 8:00 a.m. to 5:00 p.m., excluding state and federal holidays.
One advantage of submitting your request in person is that you may pay the $10 replacement permit fee directly at the service window using a credit or debit card. Unlike mailing your application, you do not need to obtain a cashier’s check, money order, or certified check in advance.
Submitting your request in person also allows you to hand-deliver your paperwork directly to AZDPS. Please allow AZDPS up to 45 days to process your request and issue your replacement permit card. Once approved, your new card will be mailed to you.
