Common Mistakes When Mailing Your Arizona CCW Renewal Application

❌ Using Colored Ink

Arizona DPS requires completing mailed applications in black ink only. It’s also listed on the application. 

❌ Writing Illegibly

Applications that are difficult to read can delay processing or require follow-up clarification. The AZDPS application is a fillable pdf file, so you may easily type the application. Click directly into the highlighted form fields to enter your information, and use the check boxes or drop-down menus where applicable. Then you may print it to sign and date for submission.

❌ Entering Your Name Incorrectly

Your legal name must be entered in the proper format as requested on the application (last, first, middle). This may cause your name to be printed incorrectly on the issued permit card.

❌ Checking Multiple Race Boxes

Only one race category should be selected. Marking multiple boxes can result in processing delays and verification.
